How do I add waivers and surveys to a class registration? - Fri - Oct 9, 2026 - 3:40pm

Waivers: In the class Settings, attach liability waivers that patrons sign during enrollment. Every active waiver on the class is required.

  • Reuse a waiver from your venue library or create a new one for the class.
  • Manage shared waiver templates under Forms > Class waivers.
  • Inactive waivers stay on file but do not block enrollment.
  • A waiver that patrons have already signed cannot be deleted. Deactivate it instead.
Surveys: Use class surveys to collect information such as allergies, T-shirt size, or experience level.

  1. Go to Forms > Surveys and create a survey with the Classes registration type.
  2. In the class Settings, link the survey.
  3. Choose how often it is collected: per enrollment, once for each participant, or per order.

Required survey questions affect each enrollment’s readiness status on the roster (Complete, Waiver pending, Survey pending, or Action required).

Required safety profile for class registration

You can now require patrons to complete a health and safety profile before they register for a class. This is useful for collecting allergies, medical needs, sensory information, and similar details up front.

To turn it on:

  1. Go to Classes > Class manager and open the class.
  2. Open the class Settings and find the Waivers & Safety Profile section.
  3. Check Make health and safety profile required to register.
When this is on, the safety profile always appears expanded during registration and is marked with an asterisk (*) to show it is required. The guardian must also complete the safety profile for themselves, not just for the participants. Patrons can’t continue to payment until it is filled out.

The information patrons enter is available in the Roster tab for the class, so staff can review allergies, medical needs, and other details in one place. The roster export may contain confidential health information, so handle it carefully.
